Move thread flags and state into 32bits.

We need to ensure that transitions to Runnable are atomic wrt to a
thread modifying the suspend count. Currently this is achieved by
holding the thread_suspend_count_lock_. This change creates a set of bit
flags that summarize that the suspend_count_ is raised and also others
flags that signify the managed code should go into a slow path.

The effect of this change are two-fold:
1) transitions from suspended to runnable can CAS the thread state
rather than holding the suspend_count_lock_. I've manually inspected all the callers and added a
ScopedHeapLock where I think one is necessary. In manual testing, this makes
jdb a lot less prone to locking us up.
Fix detaching a debugger while threads are suspended.

The interesting part of this change is in "thread_list.cc".

I've done a TODO in TagFromClass, but haven't seen it make any practical
difference in a debugger. I also tightened up the types in GetThreadStatus
while investigating the fact that we report some threads as "RUNNING, SUSPENDED",
which makes no sense until you realize that TS_RUNNING corresponds to both
our kRunnable thread state and our kNative thread state, the latter of which
may actually be a suspended thread.

I've also made us fail faster in the "address in use" jdwp failure case,
and tidied up a bunch of the capitalization in logging.

Change-Id: I0fe705791d07db31c4615addce44da4fdfbfd0d1

Fix at least two deadlocks.

Pretty much every caller that takes the thread list lock can then go on to
cause allocation, which requires the heap lock. The GC always takes the heap
lock first and the thread list lock second (to suspend/resume other threads).
Cue deadlocks.

This patch is a pretty degenerate fix that basically makes the thread list
lock irrelevant; we now always take the heap lock first.

Change-Id: I0537cffb0b841bfb5033789817793734d75dfb31
"
